I. Primary Function/General Purpose of Position
The primary responsibility of the BSMART Counselor is to assist the ER physician and on-call psychiatrist with the assessment and disposition of patients who present with a psychiatric illness in the Emergency Room setting.
The primary responsibility of the Access Responder is to provide confidential screening via telephone for psychiatric, behavioral and chemical dependency problems to Access center callers, including the person in crisis, a friend or family member, or referring professional. Responsible for managing all aspects of screening, triage, and referral services for the Access center, a 24-hour, seven-day-a-week crisis intervention, assessment, referral and admission service. Employee provides continuity of care by assisting unit in admissions/discharges and overall flow.
II. Employment Qualifications
Master's degree or a current licensed registered nurse (RN) required. Master’s degree in social work or counseling preferred.
Licensure as a Clinical Social Worker (LCSW) or Professional Counselor (LPC) preferred.
Work requires two years of demonstrated experience working with patients who have psychiatric illness, chemical dependency issues, and case management experience.
Knowledge of abnormal psychology and mental health problems.
Knowledge of appropriate clinical screening and interventions with patients.
Ability to effectively interact with persons of widely diverse roles, backgrounds, cultures, and socioeconomic classes, and with those in crisis or who are resistant or negative towards intervention.
Ability to develop therapeutic relationships with patients and their families,
Demonstrates a high level of communication, organizational skills, and assessment skills and is able to work independently.
Demonstrates knowledge of the utilization review process.
Ill. Essential Job Functions
Establishes collaborative practice with ED nursing personnel, physicians, and other community healthcare professionals regarding the assessment and disposition of psychiatric patients admitted to the ED. In addition, assists with the disposition of those patients identified as high risk by the ED healthcare team. Collaborates with the on-call psychiatrist to develop treatment goals in the immediate crisis and appropriate discharge plans for the patient. Involves patient/family in the planning process and educates them regarding care options. Investigates level of care and financial resources in formulating plans. Collaborates with post discharge care providers in the development of the transition plans. Obtains physician involvement and agreement with plan. Advocates for patient choice. Completes Case Management duties for behavioral health patients in the Emergency Departments. Initiates contact with insurance agents, providing necessary clinical information and documentation, to secure pre-authorization for care prior to admission to a Bon Secours facility. Serves as a Peer Interviewer and new hire orientation consultant. Determines medical necessity and appropriateness of each admission applying established admission criteria and communicating findings to healthcare team. Identifies factors affecting patient’s physical and psychosocial comfort and seeks consultation to achieve patient outcome. Understands data relative to patient populations and utilizes information to improve quality and reduce cost. Responsible for managing and assisting with patient placement for Bon Secours Virginia inpatient psychiatric beds. Provide Connect Care support to BHU nursing to complete appropriate MD order entry (ADT 100 orders) Answers incoming calls from consumers seeking behavioral healthcare services, obtains required screening data, provides treatment recommendations, and coordinates appropriate referrals to community providers for the needed level of care. Documents all calls in the computer. Consults with callers to problem-solve issues related to a community resources and funding sources to access care. Consults with appropriate unit personnel, leadership or community resource regarding patient placement, whether in Bon Secours’ system or community. Records all consultation requests for product line and assigns appropriate resources. Manages the bed board for psychiatric patient placement. Functions to coordinate unit admissions either from direct physician admissions or admissions from other referral sources. Manages all consultation/liason (C/L) services for psychiatric consults to the medical floors. Collect, collate and distribute monthly data relative to C/L service. Provides telephonic patient support to pediatrics, adolescents, adults, and geriatrics. Works collaboratively with peers to achieve department goals in daily work as evidenced by appropriate and timely communication which is respectful and clear. Actively participates in conferences, committees, and staff meetings. Actively demonstrates excellence in service behaviors to increase patent satisfaction. Participates in the Gallup Impact Plan for department to improve work environment. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures. Provides patient care to: pediatric, adolescent, adult, geriatric patients.
