Job Summary
The Home Health Aide provides personal care, comfort measures, and environmental services in a patient’s home as directed by the Registered Nurse (RN), Occupational Therapist (OT), or Physical Therapist (PT) to achieve the goals and objectives of a medically directed plan of care.
Essential Functions
Responds to family and patient in a courteous manner, explains job role and procedures, answers questions concerning personal care, listens actively and reports any problems involving patient care or family involvement in such care in a timely manner to the RN, OT, or PT.
Assists with or provides personal care duties as assigned by the RN, OT, or PT (hygiene, bathing, mouth care, hair care, positioning, mobility, toileting, perineal care, dressing, transferring, etc.).
Provides assistance with patient’s environmental cleanliness (linen change, patient’s personal laundry, straightening room, light vacuuming, trash disposal, bed making).
Acts as an extension to physical therapy services (progressive ambulation, range of motion, etc.) as prescribed.
Takes and documents patient’s temperature, pulse, respiration, and blood pressure and weights as assigned.
Performs or assists in treatments (sitz baths, simple enemas, assisting with self-administered medications, etc.) under the direction of an RN.
Participates in the scheduling of supervisory visits with the RN, OT, or PT.
Assists with light meal preparation, assisting the patient with meals, and reinforcing diet instruction instituted by the RN, OT, or PT.
Schedules patient visits in accordance to agency policies and procedures and calls each patient to communicate the need for an amended arrival time if running late
Job Requirements
High School graduate or G.E.D.
Virginia State Health Department for Home Health Aides and holds a CNA from VA
CPR certification.
One year (minimum) experience in a health care setting.
Valid drivers license and an acceptable driving record
Demonstrates ability to read, write, and communicate effectively
