  • Position Description Position Title: Community Health Worker Coordinator Revision Date: 03.28.2024 Department: AccessHealth Tri-County Network Reports To: Program Director Job Code: 385Q00EX Supervises: N/A FLSA Status: Exempt Primary Function/General Purpose of Position Under the supervision of the Program Director, the Community Health Worker Coordinator is responsible for providing supplemental program coordination and sound operational direction for the Community Health Worker Initiative. Drawn from the components of a framework developed by the Centers for Disease Control and Prevention (CDC) Diabetes Prevention Program, the project’s goal is to increase knowledge of pre- and post-diabetes management, hypertension, and other health comorbidities. Improve protocols for screening and referral to local program and will increase the availability of DPP in the region. The CHW Coordinator will work close with the Community Health Workers, Program Director and key consultants in coordinating this project. Essential Job Functions

  • ​​​Along with the Program Director, the CHW Coordinator will plan, coordinate, communicate, implement, monitor, and troubleshoot program activities and progress.

  • Develop centralized materials and resources repository for all materials developed by the organization and its partners through the course of the program.

  • Develop, strengthen, and maintain partnerships with local, state, and national organizations in diabetes prevention efforts. Represent organization in meetings and conferences, as appropriate.

  • Facilitate in-person, weekly, or bi-weekly sessions with several groups of participants through the year-long Diabetes Prevention Program by providing curriculum to class participants using a CDC-approved, evidence-based curriculum.

  • Plan and coordinate project outreach efforts to include the recruitment of participants, project sites, and lifestyle coaches in the community.

  • Meet regularly with the Community Care Navigation Team to discuss the pending status and ongoing conditions of patients that require supplemental programs specifically for patients of the Access Health Tri-County Network like primary care, vision/dental clinic, life skills classes, etc.

  • Identifies and coordinates with community resources that benefit the overall health promotion status of the patient and that address social determinants of health.

Licensing/Certification Lifestyle Coach Training PreferredCertification as a Community Health Worker must be successfully obtained within a specified time frame provided at the time of hire. Education Associate Degree in health care or Bachelor’s degree from an accredited college/university preferably in healthcare, health promotion or public health required Work Experience 3 years of experience in a health care or social service setting required.2+ years of experience in direct education and/or coaching with individuals and groups for disease prevention, health promotion and/or behavior change case management preferred.Experience related to the program population is strongly preferred. Training EPIC Electronic Health Record preferred Language None Patient Population Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures.​Adults (18-64 years)Geriatrics (65 years and older) Working Conditions Periods of high stress and fluctuating workloads may occur.Long-distance or air travel as needed- not to exceed 10% travel.General office environment.
