Primary Function/General Purpose of Position
This position will provide direct patient interaction and coordination of services at the direction of the Sr. Project Manager of Grants Department. Provides an excellent patient excellence during the episode of care, using extensive knowledge of patient resources in the local community to provide excellent patient experience.
Job Functions
Provides ongoing episodic community-based support and assistance to patients and staff to implement care plan as needed necessary.
Acts as a resource to patients and families, providing answers to frequently asked questions and suggesting educational material.
Maintain comprehensive records for each patient interaction.
Maintains good working relationships with and knowledge of applicable community services.
Utilize interpersonal skills and community resources to effectively advocate for individuals in the communities served.
Maintains required data and documentation and prepares required reports.
Provide education to healthcare workers treating patients with substance use disorder.
Perform duties with minimal supervision. May be required to occasionally work irregular hours, attend other job-related meetings, and perform other duties as assigned.
Collect patient information according to protocol.

Licensing/Certification
BLS Basic Life Support – American Heart Association – Preferred
Community Health Worker certification - Preferred
Education
High School or GED – Required
Associate degree, Healthcare or related - Preferred
Work Experience
6 months experience in community or ambulatory care setting with direct patient interaction.
Excellent interpersonal communication skills. Strong analytical, data management and computer skills. Demonstrate basic knowledge of healthcare and health education. Ability to work with individuals, groups, and families. Familiarity and knowledge of Community Resources. Flexibility to work non-traditional hours. Works well in team setting. Personal computer skills. Experience with database entry. Highly organized and detail oriented. Accepts responsibility and follows through on projects and activities.
