Work Locations: Position requires local travel to all department locations including BSSF Hospital (West Ashley), Moncks Corner MOB, and Mount Pleasant MOB.

Job Summary :

To provide high quality healthcare through the provision of medical nutrition therapy in a variety of settings to meet the needs of patients, their families and other healthcare professionals.

Job Summary specific to Women, Infant and Children’s Services (WIC) : Under the direction of the Department Director, the Registered Dietician/Certified Diabetes Educator will be responsible for the performance and coordination involving the assessment, evaluation, and education of patients, their families, and/or significant others. Will serve as a liaison between Maternal-Fetal Medicine and other nursing departments. Along with other team members will be responsible for the facilitation of the diabetes plan of care. The Certified Diabetes Educator will be an active participant in the patients’ case management of diabetes.

Job Summary specific to Home Health :

The Registered Dietitian/ Certified Diabetes Educator is responsible for directing, consulting and orienting all nursing and therapy staff on all nutrition and diabetic elements of patient care. Provides assessments, screenings and recommendations for patients at nutrition risk and for those receiving advanced nutrition therapies, as well as aids the professional staff in developing and implementing care plans for patients diagnosed with diabetes. Makes home visits and telephone consults to patients as the need is identified. Provides nutrition and diabetic in-services to all home care staff. Reviews all laboratory results obtained by staff and makes recommendation on nutrition interventions based on the results. Is responsible for assuring that all nursing staff is competent in the use and care of blood glucose monitors as well as aids staff in obtaining glucose monitors and supplies for patient use in the home. Is responsible for maintaining current standards of practice for nutrition and diabetic therapy as well as develops policies, procedures and protocols to meet current practice guidelines.

Job Summary specific to Bariatric and Metabolic Svcs :

To provide high quality healthcare through the provision of medical nutrition therapy in a variety of settings to meet the needs of patients, their families and other healthcare professionals. To serve our bariatric surgical and metabolic weight loss patients, through nutritional education and dietary counseling as an important member of our multidisciplinary bariatric and metabolic services team.

Job Summary specific to the Diabetes Treatment Center :

The Clinical Dietician in the Diabetes Treatment Center is responsible for directing, consulting and orienting all nursing and therapy staff on all nutrition and diabetic elements of patient care. Provides assessments, screenings and recommendations for patients at nutrition risk and for those receiving advanced nutrition therapies, as well as aids the professional staff in developing and implementing care plans for patients diagnosed with diabetes. Provides nutrition and diabetic in-services to staff. Reviews all laboratory results obtained by staff and makes recommendation on nutrition interventions based on the results. Is responsible for assuring that all nursing staff is competent in the use and care of blood glucose monitors as well as aids staff in obtaining glucose monitors and supplies for patient use in the home. J ob Summary specific to Mt Pleasant Cardiology :

To provide high quality healthcare through the provision of medical nutrition therapy in a variety of settings to meet the needs of patients, their families and other healthcare professionals. To create and coordinate a multi-disciplinary lifestyle modification program to help participants lower their risk of heart and vascular disease through healthy exercise and eating patterns.

Minimum Qualifications :

Education: Bachelor’s degree from an accredited college or university. Completion of an ADA approved Dietetic Internship Program.

Education specific to Women, Infant and Children’s Services (WIC): Bachelor’s degree in Food, Nutrition or Dietetics required.

Experience: None

Experience specific to Women, Infant and Children’s Services (WIC): One year of experience in diabetic patient education or supervision.

Experience specific to Home Health: Minimum 5 years of work experience as a dietitian and/or Certified Diabetic Educator (CDE) in a health care setting.

Experience specific to Bariatric and Metabolic Svcs: Previous experience working with a multidisciplinary bariatric surgical program is strongly encouraged.

Experience specific to Diabetes Education Program: Must have experience as a dietitian.

Experience specific to Mt Pleasant Cardiology: Minimum 5 years of experience as a dietitian in a cardiovascular-health setting. Currently registered through the American Dietetic Association. Preference for individual that is certified in Adult Weight Management

Licensure/Certification: Must be credentialed as a Registered Dietitian by the Commission on Dietetic Registration (CDR), recent graduates will be considered but must be Registered and Licensed.

Licensure/Certification specific to Women, Infant and Children’s Services (WIC): Must be a Registered Dietician by the Commission on Dietetic Registration (CDR). Must hold current certification as a Diabetes Educator by the National Certification Board of Diabetes Educators (NCBDE); or is eligible for certification and successfully obtains certification with 6 months of employment.

Licensure/Certification specific to Home Health: Currently registered through the American Dietetic Association. Currently is Certified as a Diabetes Educator by the National Certification Board of Diabetes Educators or is eligible for certification and obtains within 6 months of employment Must have a current American Heart Association BLS for Healthcare Provider Card.

Licensure/Certification specific to Diabetes Education Program: Certified Diabetic Educator (CDE) preferred. Currently registered through the American Dietetic Association.

Knowledge/Skills: Assess patients’ nutritional needs and develop and implement plans and goals for nutritional care. Ability to evaluate and assess the appropriateness and effectiveness of nutritional therapy. Ability to provide education and/or consultation of patients, family and other healthcare professionals. Ability to communicate with the healthcare team information that impacts patient care. Conducts diabetes self-management education according to best practices in both individual and group setting. Coordinates care, collects diabetes data, and serves as a diabetes resource to healthcare team.

Must be able to follow oral and written instructions and procedures and to communicate clearly.

Other: Responsible for professional growth and development, functions as a clinical resource for the interdisciplinary care team, utilizes evidence based practice to guide the provision of care. Responsible for mentoring/precepting dietetic interns as assigned. Participates in health fairs and other community activities that promote health, wellness, and CareAlliance Health Services. Serves as a resource for compliance with modified diets through direct and indirect interaction with the diet office.

Home Health: This position requires operation of a vehicle for Roper St. Francis Healthcare business. Due to this job requirement, the selected candidate/employee must possess a valid, current South Carolina driver's license. In addition, the selected candidate/employee must be insurable under the Roper St. Francis Healthcare vehicle insurance plan, which will be verified by reviewing licensure and driving record.
